Form ADV is the registration and disclosure document an investment adviser files with regulators, with Part 2A serving as the client-facing brochure describing services, fees and conflicts.

It is the document a prospect can read before meeting you and an examiner will read alongside what you actually do. The gap between the two is the finding.
